    What Is Motor Neurone Disease (MND) / Motor Neuron Disease?

    Motor neurone disease is a group of diseases that affects the motor neurones (or motor neurons) – the nerve cells in the brain and spinal cord that control movement. These motor neurons are nerve cells that send signals from your brain to your muscles so you can walk, talk, swallow and breathe.

    In MND:

    • Upper motor neurons (in the brain) and lower motor neuron cells (in the spinal cord) gradually die.
    • This loss of motor neurones leads to muscle weakness, stiffness and muscle atrophy (wasting).
    • Over time, this disease that affects the motor nerves can make everyday tasks harder – walking, climbing stairs, using hands, speaking and swallowing.

    Different forms of MND exist, including:

    • ALS (amyotrophic lateral sclerosis) – the most common form of MND, often known as Lou Gehrig’s disease.
    • Progressive bulbar palsy – affecting speech and swallowing early.
    • Spinal muscular atrophy and Kennedy’s disease – other types of motor neurone disease and neuromuscular disorders.

    Because MND is also known by different names in different countries (for example MND Australia and ALS in other places), it can be confusing – but the core problem is similar: upper and lower motor neurons connected to muscles are getting damaged.

    Types of Motor Neurone Disease

    Doctors describe types of motor neurone disease or types of motor neuron disease based on which motor neurones are most affected and how fast the disease progression is:

    ALS / Motor Neuron Disease

    Affects both upper and lower motor neurons, causing limb weakness, stiffness, and later speech and breathing problems.

    Progressive bulbar palsy

    A type of MND where symptoms include early slurred speech, swallowing difficulty and facial weakness.

    Primary lateral sclerosis

    Affects only the upper motor neurons, causing stiffness but less wasting.

    Progressive muscular atrophy

    Early symptoms of MND and symptoms of motor neurone disease can be subtle. Early symptoms and onset of symptoms may include:

    • Weak grip, dropping things, or foot dragging
    • Symptoms include muscle cramps or twitching
    • Slurred speech or soft voice
    • Weakness in arms, legs or neck

    As the disease progresses, symptoms may spread to more muscles, causing:

    MND symptoms vary between people, and disease can occur at different speeds. Some people feel stable for long periods, while others have faster progression of symptoms and progression of the disease.

    Causes of Motor Neurone Disease – What We Know and Don’t Know

    The exact causes of motor neurone disease and causes of most MNDs are still not fully understood. In many cases:

    • A mix of genetic mutation and environmental factors may be involved.
    • A small percentage are clearly inherited (family history).
    • In many people, doctors never find a single clear cause.

    Similarly, causes of ALS – the common types of MND – are not fully known. Because of this, there is currently no cure for MND or cure for motor neuron disease, and no way to completely prevent disease in most cases.

    However, treatment options available – including medication like riluzole, supportive rehab and supportive treatment – can help manage the symptoms, slow some aspects of disease progression, and improve quality of life for people living with MND.

    At present, worldwide guidelines and major national institute of neurological bodies (such as the National Institute of Neurological Disorders and Stroke) say there is no complete cure for MND or cure for motor neuron disease. However, there are treatments for MND and treatment options available which can help:

    • Riluzole and other medicines – treatment for ALS and treatment for motor neurone disease may include drugs like riluzole that can modestly slow some ALS/MND forms.
    • Symptom-based medicines – to help reduce saliva, cramps, mood changes, pain and discomfort.
